Telstra’s Triple Zero outage had three separate causes

Telstra’s investigation into the 1 March outage found that it was not caused by a single failure. CEO Vicki Brady said the incident resulted from three issues occurring together.

The first was a technical fault that caused connections to a database to reach their maximum limit. This triggered a software fault that had not been detected. The resulting disruption affected several Telstra services, including mobile, internet and landline connections across Australia.

The second problem involved the backup process used for state emergency operators. Telstra stores backup phone numbers in a secondary database so calls can be transferred manually when required. For eight of the 24 hours on 1 March, the stored numbers for the 24 state emergency operators were incorrect, preventing those manual transfers from working as intended. The third issue was a communication error that delayed some callbacks from emergency service operators, particularly in Victoria. An updated email address supplied to the team was entered incorrectly into the system.

The outage had serious consequences

The incident received significant attention because of reported delays involving emergency calls. Victorian Ambulance Union secretary Danny Hill told ABC Radio Melbourne that a paramedic crew responding to a patient experiencing cardiac arrest was delayed. The patient later died, although the transcript does not establish that the outage was the sole cause of the death.

Reports also said some crews were dispatched without complete information. In some cases, details sent by email arrived after crews had reached the scene. Telstra apologised to people who attempted to call Triple Zero during the outage, including the family of the man who suffered the cardiac arrest.

Fibre upgrade milestones in New South Wales and Victoria

NBN Co announced that one million premises in New South Wales and one million premises in Victoria were fibre-ready. This means the premises are eligible for an upgrade, rather than having fibre already connected inside every home. The ACMA was considering opening the 6 GHz spectrum band for unlicensed use, which could support technologies such as Wi-Fi 6 and Wi-Fi 7. Supporters said it could enable new wireless technologies, while the Australian Mobile Telecommunications Association raised concerns about interference with critical services and existing telecommunications infrastructure investments.

I think the Triple Zero findings show how several smaller technical and operational failures can combine into a serious outage, particularly when backup and communication processes also fail. The NBN fibre milestones are encouraging, but eligibility is not the same as a completed home connection. I also see Telstra’s Starlink offer as having a significant speed limitation for a relatively small monthly saving.
